The Pathway — Step by Step
Phase I — Hepatic Oxidation
The liver oxidizes active estrogens (estradiol, estrone) into metabolites. Some of these metabolites are protective — the 2-hydroxy pathway produces weakly estrogenic, antioxidant compounds. Others are more reactive — the 4-hydroxy and 16-hydroxy pathways produce metabolites with stronger proliferative potential. Phase I is where estrogen metabolite profile is shaped.

Phase II — Conjugation (Glucuronidation)
The liver attaches a glucuronic acid molecule to the estrogen metabolite, converting it from fat-soluble to water-soluble. This packaging is what makes elimination possible. Phase II requires adequate glutathione, B vitamins, and liver health to function efficiently.

Bile Transport to the Gut
Conjugated estrogen metabolites are moved into bile and transported from the liver into the small intestine. Adequate bile production and flow is essential — sluggish bile means conjugated compounds back up rather than moving toward elimination.

Gut Microbiome — The Disruption Point
When the microbiome is healthy, conjugated estrogen metabolites travel through the colon and exit in stool. When beta-glucuronidase-producing bacteria are elevated, they remove the glucuronic acid packaging — deconjugating the metabolites and releasing free estrogen back into circulation. This is enterohepatic recirculation.
